New Patients

Welcome!

If you or your child has an upcoming appointment with us, we want you to know that we’re looking forward to meeting with you.  As Omaha’s leading pediatric ophthalmology and adult strabismus specialists we take pride in providing excellent care for our patients and facilitating the long-term health of their eyes.

If this is your first time here, check out our Contact page for ways to get in touch with us and for directions to our office.  Then, head over to the Forms page to download the registration forms that you’ll need for your visit.  You don’t have to do this ahead of time, but if you do have your forms printed and completed when you check in we’ll most likely be able to reduce your wait time at the office.  Also, please make sure that your child’s previous medical records relating to his or her eye care (from a family doctor or other physician) have been sent to the office ahead of your visit.  You can find a medical records request form on the Forms page.  This will allow for the appointment to be as productive as possible, and help the doctors to better treat your child.  Finally, please make sure that you bring your insurance card to your visit.

If you are unable to make your appointment, we kindly appreciate a cancellation notice within 48 hours if possible.

Should you have any questions ahead of your visit, check out our FAQs or give us a call at (402) 399-9400.

Thanks, and we look forward to welcoming you in person soon!
